- State House of Representatives district
- State Senate district
- U.S. Congressional district
- the entire State of Tennessee

Download & complete this form and return it to the Election Commission with necessary fees.
Voter registration records include voters' name, address, date of birth, political affiliation, voter ID number, precinct and voting history. Voter registration records include name, telephone number, address and voting record.
